My Girlfriend Is Using My Last Name – Leo and Rebecca Buzz Question

Leo and Rebecca Buzz Question...My girlfriend has started using my LAST NAME. She used it the other day at a Restaurant for the waiting list. AND the other day she got mail with her Name on it followed by my last name. WOW, she needs to slow her roll. How do I tell her without hurting her feelings to stop using my last name as hers???
